Out-of-Network Benefits and Comprehensive Evaluations
For out-of-network benefits, we can provide a super bill for reimbursement. If you have out-of-network benefits, your insurance may reimburse you based on your coverage level.
Initial Comprehensive ADHD Evaluation with QBCheck testing up to 75 minutes $199.99 includes:
- Telehealth tests that patients can take from anywhere with computer access are available on both PCs and Macs.
- Telehealth assessment conducted with a qualified healthcare provider to review QbTest results.
- Review standard behavioral screening scales.
- Medication management and discuss different treatment options.
- Resources for other services.
- Plenty of time to answer any questions and/or concerns.
- Request labs and other diagnostic tests if needed.
- Dietary recommendations if needed.
Initial Evaluation for all other mental health concerns including already diagnosed ADHD: up to 60 minutes $175.00 (proof of records required for ADHD).
- Ages 10-65.
- QbTest may be used to evaluate the effectiveness of the current treatment.
Monthly Medication Management only $75:
- Telehealth/Virtual/Remote.
- Up to 15 minutes for medication management/refills.
- Review medication effectiveness.
- Assess for side effects or adverse reactions.
- Retest QbTest annually and/or as needed to evaluate treatment effectiveness.
- Medication refills are sent within 24 business hours from the time of request (Monday through Friday).
- Collaboration with schools, counselors, and academic coaches if needed.
- Patient portal communication.
*Computer QbTest only $100:
- Review results.
- Receive a copy of results once reviewed with provider.
Forms, authorizations, and letters by request of patient $25.
*Computer testing alone is not sufficient to diagnose ADHD.
